You don’t want to know why, but you find yourself in 3NT on a 4th best spade lead. I’m sorry, but you have to plan the play.

    The only “real” line seems to be to play for a singleton CK to drop and the diamond finesse to work – this is very unlikely.

    At the table, I would lead a heart to HQ at trick 2 and hope that EW are unclear about the spade position, or there is a blockage. I can’t see any advantage in playing the SQ under the K, as it might make it easier for the opposition to continue spades. If the HQ holds, I play the CQ and hope for a doubleton CK (or tripleton and west erring by covering). It would help to know the bidding.
